Program Adjustment Request Form – Mini-Grant
A program adjustment request form is completed when there are available grant funds from a project or supplies that you want to reallocate to other strategies or activities.
Deadline for submitting adjustments is May 31, 2017.
All program adjustment requests must be completed by the grantee and with the assistance of the Regional Support Center. Regional Support Centers will send the completed form to ACT Missouri.
Mid-Year Report Form
Submit reports on January 15, 2017 to the Regional Support Center. This report reflects the grant activities from July 1, 2016 – January 10, 2017. The Regional Support Center will complete the cover sheet and submit this report along with the cover sheet to ACT Missouri by January 27, 2017. Failure to submit this report in its entirety may result in loss of funding for future programming.
ACT Missouri Mini-Grant Final Program Report
Fifteen days after the conclusion of program activities, each funded program must submit to ACT Missouri a Final Program Report. Failure to submit this report will result in loss of funding for future programming. Final reports are due no later than June 30, 2017 (to the RSC) and to ACT Missouri no later than July 15, 2017.
